How are residents assessed and transitioned?

In addition to the assessment carried out by us, the referral process also includes inviting family, friends and the Care Manager to visit our homes to see for themselves the environment and services available. Where it is not possible or appropriate for the service user to visit in person, it is important we are guided by someone who knows the individual well and can advocate on their behalf.

Should all parties, including the service user themselves, agree that the placement is appropriate and, funding is agreed a transition plan can be devised.

A service user’s transition plan is specifically tailored to their individual needs and provides as much support as is possible to aid the person in the move to their new home.

Our thorough referral process is intended to help maximise, the possibility that the placement will be a success.
